Transition planning for youth with special health care needs and disabilities is crucial to assuring they achieve their goals and dreams for the future.

A major barrier identified by transition age youth is health care needs as it can interfere with their ability to work, live and play in the communities of their choice. Health care professionals unlike other professionals such as special education teachers and rehabilitation specialists have scant amounts of information on how to best prepare youth for the transition from pediatric to adult health care services and to other transition and adult resources for employment, training, education and community living.

The Center for Disability Studies is involved in the development and implementation of demonstration projects, generation of evidence and best practice information for youth, young adults, families, caregivers and professionals who work in health and non health settings.

The South Los Angeles Youth and Young Adult Self Determination Center

 This program is designed to provide:

· Outreach training
· Community referrals
· Technical assistance
· Facilitate interagency partnerships
· Mentorship training
· Enhance community

Project staff provide technical assistance and outreach training to youth, young adults and families about the array of transition, youth and adult programs available.

Technical assistance is provided to youth, families and providers to facilitate the transition of adolescents with developmental disabilities from pediatric to adult health care, from school to work and from family home to independent living by addressing their needs for services and supports.

Project staff assist providers of services and supports to gain valuable information and understanding about transition and adult programs, thereby serving as an interagency resource enhancing their capacity to provide services to youth and emerging leaders with developmental disabilities and their families.
